Immediately after placing your order, CPS will send you an email with a link to your test battery.

In 1 - 2 business days after you've submitted all test responses, CPS clinicians will perform an assessment, assign a diagnosis, and priority mail you the results, including the Letter of Prescription and a report with information about your diagnosis.

If you select printable documents, they will be emailed as PDF files and on the same day your priority mail package is shipped.

NOTE: Test scores, raw data, test questions/answers, and subjective results of interview questions are not available to clients. Only the specific services purchased will be provided. By purchasing services from CPS, the client acknowledges and accepts these terms.

A Letter of Prescription is Not Guaranteed Although this service includes both the disability assessment AND a letter of prescription for an Emotional Support Animal (ESA), purchase of these services does NOT guarantee a client will qualify as disabled. The CPS assessment and letter of prescription for an ESA is valid for 12 months, at which time a new assessment and letter of prescription are required. This online psychometric and psychosocial test battery provides the foundation for your assessment, which allows us to assign a diagnosis from the latest revision of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV-TR) and write you a letter of prescription. The online assessment uses 128-bit encryption to keep your responses private. The findings of our assessment, along with the DSM-IV-TR diagnosis are included in the formal clinical report we prepare and send to you. The Information Release option should be added if you plan to fly with your emotional support animal. Property managers will routinely require a tenant's mental health professional to complete a 3rd Party Verification Form to qualify the tenant. If you seek approval for housing with your ESA, the Verification Form Completion service should be added.
