Refunds and Cancellation Policy
When you order the CPS emotional disability assessment and letter of prescription service, you are immediately sent a link to the requisite test battery (on the confirmation page and via email). Once ordered, the disability assessment cannot be cancelled or refunded if the testing component has been opened or started. You aren't required to proceed with the test and assessment, but you have essentially paid to examine and/or take some or all of our testing components, whether or not you take the tests.
Refunds
If you choose to cancel the services you ordered before you open the test component, you may be refunded for your entire order, including shipping. CPS must receive a cancellation and refund request via email or telephone within 30 days of the date your order was placed.
Limited Refunds
In the event that you elect to cancel your order after opening the testing component, and before any other services/products are processed /sent, you are entitled to a refund of all services and products, except for the disability assessment itself. Since the letter of prescription is bundled with the disability assessment as a single product, the refund value of the letter of prescription has the same value as the cost of a Replacement Letter of Prescription. Other options that were added to the assessment/letter service, such as PDF copies, Info release forms, etc., may be refunded at the original cost you paid when the order was placed.
Typographical Errors
Because of the automated nature of client-entered information (the CPS test component and data system relies on client input, not CPS staff input), it is extremely rare that CPS staff or the automated system will cause an error as it relates to a client name or address. If this does occur, however, CPS will be responsible to remedy the situation immediately and, if necessary, resend the documents or other forms at no cost to the client. In the event there is a typographical error caused by CPS, a client is not entitled to a refund for services. CPS is happy to fix the problem and make it quick, but orders that have already been processed may not be cancelled or refunded.
Failure to Qualify for a Letter of Prescription
In the event that you fail to qualify for a letter of prescription for any reason after completing the CPS test battery, CPS will always contact you for additional information or clarification if there is any concern about meeting the criteria for a letter of prescription. If you are unable to provide additional information or the information you provide is insufficient to qualify you for the letter of prescription, CPS will refund you for all services paid for, except the disability assessment itself.
