Disability Assessment and Letter of Prescription
This service is a psychometric and psychosocial assessment of your current emotional or psychological state to determine if a client qualifies as disabled. This service includes a required online test battery and multiple counseling sessions. A letter of prescription for an Emotional Support Animal (ESA) is included for clients who qualify as emotionally/mentally disabled.

This service enables CPS to complete and return Third Party Verification forms for housing, apartments, college dormitories, and other situations to approve your emotional support animal.

If you order this service and have NOT ordered and completed a Psychological and Emotional Disability Assessment within the last 12 months, your order will NOT be processed.
